The North Central District Home Health Agency's objective is to provide you with quality care that meets your needs. Highly trained, skilled professionals can care for you in your home when you are ill or disabled as an alternative to a longer hospital stay or nursing home placement. The following services are provided when ordered by a physician and conditions of eligibility are met.
SKILLED NURSING
Services are provided by a registered nurse who develops a plan of care with you and your doctor. These services can include: assessment of your medical condition, wound care and dressing changes, IV therapy, catheter care, colostomy care and teaching you and your family about your illness and your care needs. The nurse coordinates your care and keeps your doctor informed of your progress.
CLINICAL THERAPY
Licensed therapists provide treatment to relieve pain, prevent further disability and restore function for maximum independence.
MEDICAL SUPPLIES
Wound care dressings, tape, catheters, and ostomy supplies can be provided.
MEDICAL SOCIAL SERVICES
Social workers help with personal, family and financial problems that may develop as a result of your illness. They can assist with referrals to community resources to help in resolving some of your problems.
HOME HEALTH AIDE SERVICES
Trained professionals can provide assistance with personal care: bathing, hair care, changing linens, meal preparation, and light housekeeping in some instances and assist with simple prescribed exercises.
HOME AND COMMUNITY BASED SERVICES WAIVER PROGRAM
Some low-income elderly and disabled I individuals are eligible for a special Medicaid program that offers an alternative to nursing home placement. This program gives some individuals the chance to remain living at home if they have the right kinds of care and support. This program provides some services that are regularly not provided by traditional home health services. These services include assessment, case management, personal care, homemaker/light housecleaning, respite care to relieve a full time caregiver for short periods
of time, and access to Adult Day Health Care Centers.

If home health services are not appropriate to meet your needs, we will make every attempt to refer you to the appropriate agency or individuals to assist you in obtaining the services you need.
If you are hospitalized and your doctor orders home health care services for you, you may request that the hospital discharge planner or social worker call the North Central District Home Health Agency to provide you with care at home.

The North Central District Home Health Agency provides professional and ancillary services to individuals in their places of residence. These services are designed to restore acutely ill, convalescing and impaired individuals to their maximum potential for good health and independent functioning. The North Central District Home Health Agency also seeks to promote understanding of good health practices: to promote physical, emotional, and psychosocial care to ill patients and their families; to prevent unnecessary institutionalization and to facilitate early hospital discharge by providing technically feasible treatment in the home where the atmosphere for healing is most beneficial to the patient.
The North Central District Home Health Agency aims to provide guidance and counseling in locating and utilizing personal and community resources to cope with problems that contribute to poor health, and to participate in the activities of local, regional, state and national organizations representing the health care industry.
North Central District Home Health Agency provides services to
individuals residing in Henry, Oldham, Trimble, and Shelby counties.
Our office is based in New Castle and can be reached by telephone
toll free from any on the four counties we serve. Our office hours are 8:00 a.m. through 4:30 p.m. Monday through Friday. Nursing services are available 24 hours a day, 7 days a week utilizing on-call nursing staff contacted through our 24-hour answering service.
