Phase 1 - Organizing for success/ partnership development *Identify the need for the MAPP process *Meet with and identify participants from the community *Design the process in such a way that best fits the community *Assess resource needs and secure commitments
Phases 2 - Visioning *Formulate vision and value statement with your community members
Phase 3 - The four assessments *Tools developed to determine: the quality of health/life in the community and the functionality of our local health system
Phase4 - Identify Strategic Issues *Review and discuss findings from MAPP assessments *Brainstorm about potential strategic issues *Determine the consequences of not addressing an issue *Consolidate overlapping or related issues *Order issues in terms of priority
Phase 5 - Formulate Goals and Strategies *Develop goals after considering the vision and strategic issues *Generate strategy alternatives if something is not going well *Consider barriers to implementation *Consider how to put goals into action *Select goals that are most likely to be achieved or are of highest priority
Phase 6 - Action Cycle *Establish accountability *Begin implementation of strategy *Coordinate with partners *Monitor how implementation plans are being carried out *Evaluate *Share lessons learned and celebrate successes |
